Absences of more than 2 days are to be reported to the health office at the numbers noted above.
HEALTH SCREENINGS
Health screenings will be conducted during your child's health class. We expect to begin screenings in October and conclude by May. All 12th grade students will receive copies of their immunization records for future reference.
IMMUNIZATION REQUIREMENTS
Any student who has not received the state mandated immunizations will be excluded from school. Transfer students must present complete records of immunizations upon registration and prior to attending classes.
Copies of immunization records and vision test records are able to be made within one school day. Requests for records after 2 PM will be processed the following school day
MEDICATIONS
No student is permitted to carry any medications during school hours. All medications must be administered by the nurse in the health office. This includes Tylenol, Advil, Lactaid, and Benadryl. Only those students who have presented a properly signed form for inhaler use may self carry the inhaler.
Please anticipate the students need for medication for headaches, cramps, body aches, or other discomforts. Yellow medication permission forms are available in the health office to be signed by a parent for over the counter medications, or by a doctor for prescription medications. This form must be properly completed prior to any medication administration. No phone consents are accepted.
All medications brought to the health office must be non- expired, clearly labeled and in an original container.
Any student needing an inhaler, epi-pen, insulin, or any medication for allergic reactions must provide the nurse with a signed Individual Health Care Plan, and a separate permission form for self carrying. These forms are available in the health office. Any student who has used these medications in the past should have received forms over the summer to be returned in the fall. Please send in any completed forms and medications as soon as possible.
PHYSICAL EDUCATION EXCUSES
Parent notes may be written to excuse a student from Phys. Ed. class for 1-3 days. For longer than 3 days, a Doctor’s note is required. Students must be prepared to dress for Phys. Ed. class when excused less than 1 week.
INDIVIDUAL HEALTH CARE PLANS (IHCP)
Students who have specific medical problems that might need nursing intervention and special teacher consideration need to have an IHCP on file for each school year. Parents need to review these plans with their medical Doctor, and return them, signed and dated, quickly. Changes can be made in these plans at any time. Students who had medical conditions needing an IHCP last year need to update the plan for this school year. An IHCP can be initiated at any time during the year, as new medical needs arise. Please contact the health office with questions or concerns.
SPORTS INJURIES
All sports injuries and all medical notes must be submitted to the health office within 24 hours of the injury. The Athletic trainer works with the nurses, and submits all medical notes to our office for the completion of insurance and accident reports. In general, the time limit for submitting insurance claims forms is 60 days from the date of injury.
SPORTS PHYSICALS
All student athletes must have a sports physical completed within 365 days of the first practice of their sport. Proper forms are available from each coach at the pre-season meeting, or any time in the health office.
Part “B” is the form for the physician to record health findings. These forms must be properly signed, stamped, dated and completed, or they will be returned, causing a delay in the student’s start of practice.
Part “A” is a health form to be completed each sports season by the parent. This must be signed and dated by both the student and parent, and fully completed, or it will be returned.
